The disguise of madness tells the story of Iselzo Urraca in his native land The Dominican Republic and briefly talks about some of his relatives. Iselzo met Yina when he was very young. He loves her, hates her then loves her again. The story involves the magic of a small town and the city. Iselzo meets a politician named Jorge who he later finds out that Yina knows too. Both Iselzo and Yina meet Jorge's wife in separate paths. The four of them fall in a benefit-amorous circle without knowing. Or do they?

The book becomes a race as Iselzo, the politician, and the politician's wife all want Yina in their lives. The politician's wife takes steps to introduce a law in congress so that she can marry the girl she supposedly loves. Will Iselzo be able to compete against the politician and his wife?

Iselzo struggles with issues that can be seen in the way he talks and some of his actions. His character is mean, intuitive, sadistic and out of this world. The disguise of madness talks about family issues, prostitution and how Iselzo makes money out of it. Another woman that he claims to love makes him like her pimp and he uses the money to get to Yina. The book talks about the greatness of the land where the story takes place, the sadness of leaving what one loves behind and the hurt caused by having to see it again especially when you are unable to keep it. The corruption in the system, economical hierarchies, bitterness and romanticism are all over the book.
